We stand firmly against individual and systemic discrimination and racism as per the Canadian Human Rights Act.\nPosition Summary and Scope of Service\nThe Team Leader (TL) is responsible for the overall care and service provided to people within assigned worksites. The Team Leader ensures that services are provided in a ways that:\noptimizes dignity, self-determination, and attainment of personal outcomes;\nmaximizes normal environmental and lifestyle features utilizing social role valorization concepts;\nsupports people to obtain best possible health; and\nsupports people to access education, training, and work opportunities\nThe Team Leader supervises direct support professionals within the assigned residences. The Team Leader develops and supports good team communication and collaborative working relationships by spending time in homes, making observations and providing feedback. Team Leaders need to understand the scope of each team members’ potential contribution and assist each team member to achieve their full contribution. The Team Leader may be required to provide direct support in the case of urgent or emergency situations.\nQualifications\nEducational:\nPost-secondary certificate, diploma or degree in a relevant social science or human services field is preferred\nCertification in CPR and Emergency First Aid\nCertification in Non-Violent Crisis Intervention is an asset\nFormal training in Personal Outcome Measures is an asset\nSuccessful Completion of Core Supervisory Training\nProfessional:\nMinimum of four years’ experience providing services to people with disabilities in a community setting\nPrevious experience as a manager in an interdisciplinary environment\nPrevious experience in a unionized environment\nKnowledge of care, education and training of persons living with a developmental disability\nKnowledgeable of the history of services to people with disabilities and current trends, philosophies and practices concerning their care, education and lifestyles\nKnowledge of the structure, regulations, and legislation of Family Services, Child and Family Services, and Residential Care Licensing is an asset\nKnowledge of theory of Social Role Valorization and Personal Outcome Measures is an asset\nPrevious experience managing a budget within a non-profit setting is an asset\nMust understand and support the concept of interdisciplinary teamwork\nAbility to plan and implement individual support plans\nPersonal:\n\nAbility to work with minimal supervision as well as a collaborative team player in a team environment\nAbility to organize and prioritize work in a fast-paced environment\nAbility to generate ideas to improve work procedures and contribute to an environment that fosters growth, development and innovation\nSuperb interpersonal ability to establish and maintain positive and productive working relationships with humility and self-awareness\nAbility and willingness to give and receive honest, balanced feedback\nDemonstrates competence and character that inspires trust\nActs with integrity and respect in all interactions\nValues and commitment to continuous self-growth and professional learning\nSupports the concept of interdisciplinary teamwork\nAbility to communicate effectively, both verbally and in writing\nFunctions effectively as a team leader and team member\nPossesses the emotional stability necessary to serve as a role model regarding cooperative teamwork, conflict resolution and process improvement\nAbility to anticipate issues/concerns and develop proactive strategies to minimize negative impacts\nDuties and Responsibilities\nDuties include:\nIdentifies and matches staff and people supported to help them explore relationships and community activities (This is known as an “exploring pair”)\nMaintains and reviews records related to Personal Outcome Measures for people we support, assessing where engagement could be enhanced\nProvides feedback to employees in their roles in an exploring pair, and in supporting Personal Outcome Measures\nProvides direction to direct support professionals within assigned residences/service areas to ensure services provided are of the highest quality and are directly related to the individual’s assessment of their personal outcomes\nEstablishes and maintains effective working relationships with people supported and their families, volunteers, St.Amant staff and external collaterals\nEnsures each person’s personal outcomes, needs and preferences are reviewed by an interdisciplinary team regularly, and the process and outcomes are documented and shared with all team members\nEnsures that the people we support are supported in accordance within the principles of the Vulnerable Persons Act, reflecting the right of all people to make their own decisions and have help, where necessary, in a manner that respects their independence, privacy and dignity\nResponsible for chairing person-centered planning meetings\nAssists Coordinator to ensure coordination and integration of services\nProvides recommendations to the Coordinator regarding the appropriate complement of staff\nCollaborates with Family Services and/or Child and Family Services to ensure person’s needs are identified and addressed, and is responsible for following up on needs, steps and goals of each person\nProvides direction and leadership to direct support staff in crisis situations, identifies, assesses and intervenes as necessary to manage crisis situations\nPromotes awareness of people’s rights and responsibilities in a least restrictive support model\nThe above information is intended to describe the general nature of this position and is not considered to be a complete statement of duties and responsibilities.
