You will prepare team members to deliver consistent, person-centered care through onboarding, role-specific orientation, core competencies, contract and regulatory training, continuing education, and competency validation. Working with program leaders, you will align curriculum with care management tools, workflows, documentation standards, and workforce performance needs.\nThe Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living. Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.\nWhat You'll Do:\nDevelops and maintains the training strategy, curricula, educational resources, and training calendar for onboarding, role-specific orientation, continuing education, and contract and regulatory requirements; monitors completion within required timeframes\nLeads staff training, onboarding, and competency validation\nProvides ongoing education, coaching, and remediation to support workforce readiness and person-centered care\nPartners with program and Quality Improvement leaders to align training with operational priorities, address performance and audit findings, and evaluate training effectiveness\nSupports implementation of new workflows, policies, tools, and contract requirements\nMaintains training and competency records to support documentation standards, audit readiness, and compliance\nStays current on care management practices, healthcare regulations, contract requirements, and learning technologies to strengthen training programs\nPerforms miscellaneous duties and special projects as assigned\nRequired Skills and Experience:\nBachelor’s degree in nursing, healthcare, education, public health, social work, or a related field, or an equivalent combination of education and experience\nCurrent Illinois RN license or other applicable healthcare provider licensure\nFive years of healthcare, clinical operations, care management, workforce education, or related experience, including person-centered care and care coordination\nExperience developing, delivering, and evaluating curricula, competency assessments, educational materials, and performance-improvement training\nDemonstrated leadership, facilitation, coaching, communication, cross-functional collaboration, and change implementation skills\nKnowledge of healthcare documentation standards, regulatory and contractual requirements, quality improvement, and corrective action processes\nValid driver’s license\nPreferred Skills and Experience:\nMaster’s degree in a related field\nExperience with Medicaid, managed care, behavioral health, social determinants of health, community-based services, care transitions, complex populations, or trauma-informed and culturally responsive approaches\nExperience with adult learning, instructional design, virtual education, learning management systems, or competency-based education\nBilingual in English and
