Working with internal teams and external stakeholders, you will coordinate initiatives that support successful transitions to community-based settings, identify access barriers, incorporate member and stakeholder feedback into program improvements, and track progress toward contractual requirements and program goals.\nThe Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living.
