You will work collaboratively with Care Managers to facilitate member transition and community living, housing navigation, resources, and funding options. You may also complete review of person-centered care plans regarding services needed based on set guidelines.\nThe Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living.
