Under the direction of the Practice Manager, Advanced Practice Provider for the Mobile Clinic, and Facilities, the Mobile Unit Coordinator functions as a Patient Service Representative and Driver supporting mobile clinic operations. This role provides non-clinical support to patients, providers, and care teams while helping improve patient outcomes in a diverse and vulnerable population. Responsibilities include patient registration, scheduling, rooming support, coordination of services, and operation of a 40-foot mobile clinic.\nDuties/Responsibilities:\nPerform Patient Service Representative duties including patient registration, scheduling, check-in/check-out, and information gathering.\nEducate patients on insurance enrollment processes and verify coverage details.\nEnter and maintain accurate patient information in the electronic health record in compliance with HIPAA.\nSupport care delivery on a mobile clinic with two exam rooms, including patient flow and basic care coordination.\nDrive and operate a 39-40 foot mobile clinic to designated service locations across the Greater Rochester and Southern Tier regions.\nConduct daily vehicle checks and coordinate maintenance in collaboration with Facilities, including waste management and supply stocking.\nEnsure the mobile clinic is secure, alarmed, and properly set up at the end of each shift.\nCreate and distribute monthly mobile unit schedules in collaboration with leadership.\nCoordinate and organize community outreach events, including vaccine, testing, and primary care initiatives.\nMaintain relationships with community stakeholders to support mobile clinic operations.\nOrient and support new mobile clinic team members.\nComplete accurate and timely documentation per organizational policies.\nDemonstrate teamwork and effective communication across departments and care teams\nMiscellaneous\nRequires the ability and commitment to respect and support patients, other employees and visitors with various lived experiences.\nRequires individual demonstration of commitment to the mission, vision, One Trillium values, behaviors and modeling them in the organization.\nResponsible for maintaining confidentiality of all patient, client, employee, protected and proprietary information.\nEmployees are accountable for meeting the performance standards of their departments and must participate as requested in compliance audits and quality improvement plans.\nRequired to participate in continuous improvement of job responsibilities and organizational or work processes to advance the organization or department. \nOther specific duties as assigned.\nRequired Skills/Abilities:\nUnderstanding of electronic health records (EHR) systems required; Epic experience preferred \nDemonstrated ability to work effectively in a team environment\nStrong interpersonal and communication skills (written and verbal)\nProficiency in computer systems and basic office software\nValid, clean New York State driver’s license required and ability to operate the mobile clinic vehicle\nEducation and Experience:\nMinimum 1 year of experience working in a medical office setting required \nMinimum 1 year of professional experience driving large mobile units required; Class B CDL license preferred\nExperience working in community-based healthcare settings preferred\nPhysical Requirements:\nWhile performing the duties of this jobthe employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs.
