You will assist with collaboration between patients (and family) and the healthcare team. You will also help to facilitate and ensure effective individualized plans of care and be responsible for tracking outcomes and addressing or troubleshooting any barriers to success. You will serve as the subject matter expert for care management, population health, utilization management, and value-based care strategies.\nIf you are passionate about supporting patients and collaborating with a diverse team to provide quality care, a RN Care Manager Lead position could be a great fit!\nMINIMUM QUALIFICATIONS:\nBachelor’s degree in nursing required; Master’s degree preferred. \nCurrent Registered Nurse license in primary state of employment, maintained per organizational and state requirements. \nMinimum five years of clinical nursing experience required. \nMinimum two years of care management or case management experience required. \nKnowledge of nursing practice across the adult care continuum (ambulatory, population health, medical-surgical, home health, post-acute, or related settings). \nAbility to coach, mentor, and support colleagues without direct supervisory authority. \nStrong organizational, communication, critical-thinking, documentation, prioritization, and problem-solving skills. \nPREFERRED QUALIFICATIONS\nExperience with population health, value-based care, ACOs, APMs, risk-based contracts, or utilization management preferred. \nCCM, ACM, or other care management certification preferred. \nBilingual or multilingual skills preferred. \nTraining in motivational interviewing, health coaching, self-management support, or related methodologies preferred. \nHIGHLIGHTS AND BENEFITS:\nWhen Saint Alphonsus takes care of you, you can take better care of our patients.
