Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcomes.\nMinimum qualifications:\nBachelor’s degree in Nursing\n\nCurrent State of Michigan Registered Nursing License. CPR or BCLS Certified.\n\nThree years of experience in an acute care setting. One year of experience in medical, surgical or radiation oncology (for oncology specialty).\n\nWhat the Nurse Navigator will do:\nCompletes a comprehensive health assessment of the patient’s physical, psychological, social, environmental, financial and functional status in collaboration with multidisciplinary team members. Includes assessment of past and present treatment course.\n\nAssists in organizing and coordinating timely patient appointments.\n\nUnderstands and develops goals, treatment plan and provider options in concordance with evidence based standards and clinical trials.\n\nWorks directly with physician lead to review diagnostic studies, clinical history and course of treatment to identify most appropriate next course of care.\n\nCollaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for patients.\n\nUses critical thinking and problem solving skills to analyze and synthesize data and formulate an appropriate case management plan to address the patient’s individual and family needs.\n\nOur Commitment\nRooted in our Mission and Core Values, we honor the dignity of every person and recognize the unique perspectives, experiences, and talents each colleague brings.
