This role assists with patient discharge planning, coordination of post-acute services, documentation management, insurance and referral processes, and communication between patients, families, physicians, and community agencies. The Senior Coordinator prioritizes multiple tasks simultaneously, ensures timely follow-up on discharge needs, and supports efficient patient throughput while maintaining excellent customer service and attention to detail. Strong organizational skills, adaptability, and the ability to thrive in a demanding healthcare setting are essential.\nSenior Coordiantor for Case Management\nThe Senior Coordinator for Case Management would work under the direction of the RN Care Managers and Social Workers. This position functions with his/her peers and other care providers for problem solving and facilitating in-patient and post hospitalization care. And coordinate, oversee records and transmit information pertinent to the resource management of patients.\nJob Qualifications\nAssociate's degree or High School Diploma and equivalent relevant experience required. Bachelor's degree preferred.\nMedical assistant or Licensed Practical Nurse (LPN) highly preferred.\n5-7 years of customer service, Insurance knowledge medical assistance or secretarial experience preferred. Prior experience in a medical setting required\nAbility to organize and utilize work hours effectively and with minimal supervision\nMedical terminology is a must\nMust be able to use Outlook, EPIC, Teams, Faxing, and phone knowledge. \nEssential Responsibilities\nCommunicate information received from payers to utilization review nurse.\nTransmit continued stay reviews and track authorizations\nComplete HENS/PASSR\nScheduling PCP/follow up appointments\nFaxing and phoning agencies and facilities to assist with discharge referrals and continuity of care\nAssist with delivery of charity items-clothing/DME/meal cards, etc. and complete transportation application and arrange transportation as needed for patients at discharge\nCompleting PCS forms\nCompiling packets for transport\nDME: Completes form, get the script, and call liaison to follow up to ensure the patient receives before leaving.\nSending referrals through Epic CareLink, Facilities, and IV infusion\nCalling facilities checking on patient acceptance\nFaxing paperwork to facilities\nPrinting documents from Epic\nProblem Solving and Multi-tasking is a must quality \nBuild relationships with Community Partners \n*This is a full-time position at Mount Carmel St. Ann's and supports the North Region to include MCNA and MCD.\nMount Carmel and all its affiliates are proud to be equal opportunity employers.
