Part-Time Day Shift Social Worker

Days (32 hours per pay period)

At Glacier Hills, we believe every resident deserves to feel supported, valued, and heard. We're looking for a compassionate Social Worker to join our team and make a meaningful difference in the lives of the older adults we serve within our Skilled Nursing community.

In this role, you'll build trusting relationships with residents and families, helping them navigate important transitions and providing support when it matters most. You'll work alongside a caring interdisciplinary team to advocate for resident needs, coordinate care, and ensure each individual receives the resources and guidance needed to thrive.

What You'll Do:

• Assist with resident assessments, care planning, and discharge planning.
• Conduct cognitive and mood evaluations and coordinate follow-up services as needed.
• Provide emotional support, guidance, and education to residents and families.
• Connect residents with community resources, financial assistance programs, and social service agencies.
• Maintain timely documentation and collaborate with the care team to support resident well-being.

What You'll Bring:

• Licensed Social Worker (LSW) in the State of Michigan.
• Bachelor's or Master's degree in Social Work or a related human services field.
• Healthcare social work experience preferred.
• Experience working with older adults or in long-term care preferred.
• A compassionate, resident-centered approach and strong communication skills.
