You'll work alongside a caring interdisciplinary team to advocate for resident needs, coordinate care, and ensure each individual receives the resources and guidance needed to thrive.\nWhat You'll Do:\nAssist with resident assessments, care planning, and discharge planning.\nConduct cognitive and mood evaluations and coordinate follow-up services as needed.\nProvide emotional support, guidance, and education to residents and families.\nConnect residents with community resources, financial assistance programs, and social service agencies.\nMaintain timely documentation and collaborate with the care team to support resident well-being.\nWhat You'll Bring:\nLicensed Social Worker (LSW) in the State of Michigan.\nBachelor's or Master's degree in Social Work or a related human services field.\nHealthcare social work experience preferred.\nExperience working with older adults or in long-term care preferred.\nA compassionate, resident-centered approach and strong communication skills.\nIf you're passionate about helping others, creating meaningful connections, and supporting residents through some of life's most important moments, we'd love to welcome you to our team.
