Monday to Friday, 8:30am to 5:00pm. No weekends. No evenings.\nWho You’ll Support\nWesley by the Bay is a HUD-assisted senior apartment community run by United Methodist Communities. Residents live on their own. As their Service Coordinator, you are the person who makes sure they know about, and can get to, the health, home care, and community services that keep them living independently.\nWhat You'll Do\nConnect residents with community resources, services, and programs that support independence and well-being\nProvide regular outreach and support to residents, including those returning from hospital or rehab stays\nHelp residents and families navigate care options and plan for changing needs\nMaintain resource information, resident records, and required reports\nCoordinate educational, wellness, social, and enrichment programs that keep residents engaged\nCreate resident communications and serve as a liaison between residents and community leadership\nPartner with property management to support resident success and access to available resources\nWhat You’ll Need\nRequired:\nHigh school diploma or GED\n3 years of experience working with older adults or people with disabilities, or in social services in a healthcare setting\nPreferred:\nBachelor’s degree\nBefore you start: a drug test, background check, and flu vaccine are required.
