Care Coordinator

Overview

Assure Resident Care Services are provided in compliance with Victorian Senior Care Policy, DMA Clinical Coverage Policy 3L and NC DHSR Licensure requirements. Have a working knowledge of Adult Care Home Policies and Procedures and NC DHSR requirement for the operation of an Adult Care Home. Reports to the Executive Director/Administrator.

Qualifications

• NC Licensed Practical Nurse or NC Licensed Registered Nurse in good standing with the NC Board of Nursing
• Must have a current, unencumbered license in North Carolina and with two years of clinical or direct patient care experience working in a health care, home care or long-term care setting.
• Prefer working knowledge of Personal Care Services and the 3L Policy or experience in assisted living.
• Prefer working knowledge of Adult Care Home Policies and Procedures and NC DHSR requirement for the operation of an Adult Care Home.
• Must have a valid NC Driver's License.
• Must have or obtain Medication Aide and medication aide training within 30-days of hire.
• Must be able to Lift, Push & Pull a minimum of 30 Pounds
• Must be able to Bend, Stoop, Squat, and reach overhead as needed.
• Must be able to read, write, understand and follow directions
• Must have no substantiated findings on the NC Personnel Registry or Health Care Program
• Must have a clear SBI and DMV record and maintain them during period of employment
• Must have a two-step tuberculin skin test (first test prior to hire and second test within two weeks of first test)
• Must be able to relate positively to residents and families and to work cooperatively with other employees, and vendors
• Preferred 1 year of experience working with residents who have dementia or a dementia related diagnosis.
• Preferred 2 years of supervisory/management experience in an Adult Care Home or similar setting.
• Must be CPR Certified.
