Are you an LPN looking for a hospice role that combines clinical knowledge, care coordination, and patient support? Vitality Hospice & Palliative Care is looking for a compassionate, organized, and dependable Licensed Practical Nurse (LPN) to join our team as a Clinical Team Coordinator in our Allentown PA, office.
Vitality is a clinician-owned and operated hospice organization where our team works together to provide compassionate, patient-centered end-of-life care. In this role, you'll be an important part of keeping patient care organized and ensuring communication flows smoothly between patients, families, providers, facilities, and our interdisciplinary hospice team.
A current LPN license is required for this position. This is a full-time, on-site position in Allentown, PA
What You'll Do
As our Clinical Team Coordinator (LPN), you will help coordinate hospice services and ensure patients receive timely, appropriate care based on their individualized plan of care.
Responsibilities include:
- Review incoming hospice referrals in collaboration with the Intake Coordinator and Director of Clinical Operations
- Communicate with patients, families, referral sources, physicians, and facilities regarding hospice services and patient needs
- Assist with coordinating the hospice admission process
- Coordinate day-to-day clinical needs with members of the interdisciplinary hospice team
- Process and track admissions, recertifications, changes in level of care, changes in condition, and live discharges
- Monitor infection and incident logs and ensure required documentation is completed
- Coordinate DME, medications, pharmacy needs, wound care supplies, and other patient needs with the RN Case Manager
- Contact physicians and facilities to obtain and verify patient orders
- Provide education and support to patients and caregivers as appropriate
- Maintain timely, accurate clinical documentation
- Assist with routine and PRN patient visits when needed

Qualifications:
- Current and active LPN license - required
- Valid driver's license, reliable transportation, and automobile liability insurance
- Strong communication and interpersonal skills
- Excellent organization and time-management skills
- Ability to prioritize multiple patient and team needs in a fast-paced environment
- Ability to work independently while collaborating with an interdisciplinary team
- Compassion and empathy for patients and families facing serious illness and end-of-life care
- Ability to follow clinical instructions and complete documentation accurately and on time
- Hospice, home health, palliative care, geriatrics, or other relevant healthcare experience is preferred
