As part of a growing organization, this individual will help to facilitate the patient enrollment process by establishing positive relationships with the teams and the patients and families we serve. This individual may be asked to assist in direct outreach efforts, reaching out to potential clients to educate about our offerings and facilitate their enrollment. The ideal candidate is personable and professional in phone interactions, adept at reviewing and understanding medical charts and has a customer centric approach. The ideal candidate has effective communication, organization, technology, and relationship building skills. Prior experience within healthcare settings is preferred.\nThis position is a 1099 contract assignment with the possibility of conversion to full-time employment. At this time we are only considering applicants in the following states: Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, and Washington.\nWhat you’ll do\nFacilitates intake, admission, and can assist with outreaching if needed, for incoming patients.\n\nProvides accurate and ongoing assessment of patient’s status in the admission.\n\nProvides administrative computer support by maintaining databases, logs, and census sheets; updates and maintains resource/contact lists, schedules and other department specific information sheets.\n\nCompletes insurance eligibility checks and assigns clients to the Clinical Outreach Specialist.\n\nWorks cross-functionally with sales, outreach, care delivery and RCM to facilitate the enrollment workflow.\n\nMaintains all the documentation involved with the enrollment process by recording all interactions in an operating system.\n\nParticipate in all staff, census and other meetings.\n\nWeekly meetings with the Outreach Team, and others.\n\nMeet or exceed established departmental KPIs\n\nAssumes other duties in support of organizational needs.\n\nAnswer incoming calls and emails\n\nYour experience and qualifications\nMinimum of 2 years of previous intake or admission experience in a high call volume environment.\n\nCMA/CCMA certificate issued by a recognized group.\n\nPrior experience in a remote role is important. You must have a quiet, secure workspace to ensure member privacy.\n\n2+ years of experience in palliative care, home health, hospice, and/or hospital discharge planning or case management, highly preferred.\n\n2+ years of experience working within or directly with one or more of the large safety net hospitals in the region, preferred.\n\nWilling and able to work Monday-Friday 8:30am-5:00pm Pacific Time, with the possibility of evenings and weekends.\n\nStrong understanding of cultural competency with the target population.\n\nMedical or business office education or training desired.\n\nExcellent oral and written communication skills required.\n\nMust have general computer skills and a working knowledge of the internet and the ability to independently learn and use Google Suite and Salesforce.\n\nTraining as or alongside social workers, nurses, medical assistants, chaplains, medical scribes, or other educational or work experiences that have led to or demonstrate fluency in clinical language and settings.\n\nProject development skills and ability to work independently.\n\nBilingual (English/Spanish), highly preferred.\n\nAdditional Information\nThe hiring process for this role may consist of applying, followed by a phone screen, online assessment(s), interview(s), an offer, and background/reference checks.\n\nBackground Screening: A background check, which may include a drug test or other health screenings depending on the role, will be required prior to employment.\n\nJob Description Scope: This job description is not exhaustive and may include additional activities, duties, and responsibilities not listed herein.\n\nVaccination Requirement: Employees in patient, client, or customer-facing roles must be vaccinated against influenza.
