In this role, you will deliver person-centred recovery planning and case management across all stages of the recovery journey.\nYou will engage with service users from initial assessment and early engagement through to sustained recovery, ensuring holistic, tailored support that promotes long-term positive outcomes. Working as part of a multidisciplinary team, you will build strong, trusting relationships, empower individuals to set and achieve their goals, and support them in overcoming barriers to recovery.\nThis is a rewarding opportunity to make a meaningful difference by helping individuals regain independence, improve wellbeing, and achieve lasting recovery within a supportive and collaborative environment.\nKey Responsibilities\nManage and coordinate a caseload of service users, developing and reviewing recovery plans and interventions\nCarry out triage, screening and comprehensive assessments\nDeliver one-to-one and group psychosocial interventions\nComplete risk assessments, safeguarding plans and case management documentation\nSupport service users to access treatment, recovery and community resources\nDeliver harm reduction interventions and promote recovery-focused outcomes\nMaintain accurate and up-to-date records using online case management systems\nSupport prison in-reach and positive community reintegration where required\nContribute to a positive team culture and uphold Waythrough’s values of kindness, courage and respect\nAbout You\nWe welcome applications from candidates with experience in substance misuse, criminal justice, mental health, social care or related sectors.
