This is more than just a job - it’s a chance to build meaningful relationships and create lasting change.\nAbout the Role:\nThis role will support the Outreach Coordinator in leading the delivery of an effective street outreach service across Bradford, ensuring people sleeping rough or at risk of rough sleeping are identified, engaged and supported into appropriate accommodation and services. The Deputy Coordinator will provide day-to-day operational support, oversee outreach activity, and help develop strong partnership working to achieve positive outcomes for service users.\nOutreach Delivery:\nSupport the planning and coordination of daily outreach activities across Bradford.\nUndertake regular outreach shifts, including early morning, evening, and welfare visits where required.\nEnsure rough sleeping reports are responded to promptly and effectively.\nLead on engagement with individuals who are rough sleeping, sofa surfing, or at risk of homelessness.\nPromote a strengths-based, trauma-informed approach to support.\nCase Management:\nSupport complex casework and provide guidance to outreach workers.\nEnsure assessments, support plans, risk assessments and case notes are completed to a high standard.\nMonitor outcomes, including accommodation placements, reconnections, and sustainment of housing.\nAssist with safeguarding concerns and multi-agency risk management processes.\nTo Succeed in This Role, You’ll Need:\nEssential:\nNVQ Level 3 in Health & Social Care (or above or equivalent) or willing to work towards this in post. \nUnderstanding of and ability to implement interventions including brief interventions MI, PSIs, CBT and ITEP. \nExperience delivering 1-2-1 and group therapeutic interventions.\nExperience of working with carers and families.\nDriving licence and access to a vehcile.\nDesirable:\nRelevant professional qualification e.g. addiction studies, counselling, social work, therapeutic qualification. \nWorking with models of service delivery and issues relating to recovery planning.\nWorking with best practice frameworks e.g. NICE, DOH, NTA, NMC, Care Quality Commission & Caldicott Standards.\nExperience carrying out assessment and recovery planning and risk management plans. \nKnowledge of working within Safeguarding and Hidden Harm.\nExperience of using Management Information Systems.\nTo view the full job and person specification, please click here.\nWhat We Offer\nWe value the people who make a difference every day.
