You will guide a multidisciplinary team to deliver consistent, culturally safe, and equitable services across the network. Access and Choice Team Members provide immediately accessible, evidence-based interventions to individuals and their families to help improve mental health and physical wellbeing.\nAs fully integrated members of the practice team, they identify health concerns and co-create practical, achievable plans with patients.
