You will provide practical, hands-on support that assists people to build confidence, develop self-management skills and connect with their communities.\nWorking as part of an integrated team, you will help ensure participants receive consistent, high-quality support while maintaining accurate records and documentation that contribute to positive outcomes.\nKey Responsibilities\nSupport participants to develop and work towards their recovery goals and aspirations.\nProvide practical, strengths-based support that promotes independence and wellbeing.\nFoster connections with families, communities and natural support networks.\nComplete recovery plans, assessments, file notes and program documentation.\nCollaborate with colleagues and external stakeholders to deliver coordinated support.\nTravel across the region to meet participant needs\nAbout You\nYou will be passionate about supporting people experiencing mental health challenges and committed to person-led, recovery-focused practice.\nProvide practical, strengths-based support tailored to individual goals and needs and facilitate programs and promote hope, recovery and self-determination.
