We’re looking for a Care Coordinatorto join our team on aFull-time\nABOUT US:\nIndiana Treatment Centersin partnership with Ascension Recovery Services is dedicated to assisting individuals on their path to recovery from substance use disorders and related mental health challenges.\nOur vision?To provide compassionate care, break down barriers, and empower individuals on their journey to recovery.\nPosition Overview\nThe Care Coordinator plays a critical role in ensuring a smooth, compliant, and patient-centered experience from admission through discharge and transition of care. This role works closely with Admissions, Clinical, Medical, Recovery Support, and Utilization Review teams to coordinate services, support documentation, and remove barriers to care.\nThis position is intentionally flexible by design. Depending on census, staffing, and program priorities, the Care Coordinator may focus primarily on intake coordination, discharge and aftercare planning, or a combination of both. Responsibilities may shift over time to support operational needs while maintaining high-quality patient care.\nEssential Duties & Responsibilities\nIntake & Admission Coordination (as assigned)\nSupport intake workflows by coordinating assessments, documentation, and internal handoffs.\nPartner with Admissions, Nursing, and Clinical teams to ensure timely, accurate intake processes.\nEnsure required consents, screenings, and documentation are completed in accordance with policy and regulatory standards.\nServe as a welcoming, trauma-informed point of contact for patients during the admission process.\nDischarge & Aftercare Coordination (as assigned)\nCoordinate discharge planning in collaboration with clinical and medical teams.\nArrange aftercare services, referrals, appointments, and follow-up care.\nFacilitate warm handoffs to outpatient providers, residential programs, or community resources.\nSupport patients and families (as appropriate) in understanding next steps and ongoing care plans.\nOngoing Care Coordination\nServe as a coordination point for assigned patients throughout their stay.\nMonitor engagement, attendance, and care milestones; escalate concerns as appropriate.\nParticipate in treatment team meetings and contribute to continuity-of-care discussions.\nCollaborate closely with Utilization Review to support authorizations and continued stay needs.\nDocumentation, Compliance & Quality\nComplete and maintain accurate, timely documentation in the EMR.\nAdhere to HIPAA, confidentiality, and all state, federal, and accreditation requirements.\nFollow organizational policies, workflows, and professional standards.\nSupport a trauma-informed, person-centered, recovery-oriented environment at all times.\nQualifications\nRequired\nHigh school diploma or equivalent.\nExperience in behavioral health, substance use disorder treatment, healthcare coordination, case management, or a related field.\nStrong organizational skills and ability to manage multiple priorities in a fast-paced environment.\nExcellent communication skills, both written and verbal.\nComfort working in EMRs and clinical documentation systems.\nPreferred\nAssociate’s or Bachelor’s degree in social work, human services, psychology, or a related field.\nExperience in residential or inpatient SUD treatment settings.\nFamiliarity with intake, discharge planning, or care coordination workflows.\nKnowledge of trauma-informed care and recovery-oriented systems of care.\nKey Competencies\nPatient-centered and service-oriented mindset\nStrong follow-through and attention to detail\nFlexibility and adaptability\nProfessional boundaries and ethical judgment\nCollaborative, team-based approach\nWork Conditions / Physical & Mental Demands\nEnvironment:\nIndoor residential treatment setting with moderate activity level.\nPhysical:\nFrequent standing, walking, bending, and lifting up to 50 lbs. with assistance.\nAbility to assist clients in physical tasks and respond to emergencies.\nCognitive/Emotional:\nAbility to prioritize multiple tasks in fast-paced settings.\nMaintain professionalism with clients experiencing withdrawal, emotional distress, or crisis.\nSustain focus for documentation and monitoring responsibilities\nWhy You’ll Love Working Here\nThe work we do truly changes lives.
