Additionally, the center serves as a curfew center for youth from 9 PM to 2 AM, offering support and services to those brought in by the police after curfew hours.\nFor more information, visit Xiente's website at www.xiente.org.\nAbout the Role\nThe Intensive Prevention Services (IPS) Case Manager supports at-risk youth in achieving social and academic success through comprehensive, intensive programming. The Case Manager helps prevent at-risk youth's entry or re-entry into the juvenile justice system. You'll work closely with an IPS Counselor as part of a small team, ensuring each youth gets tailored, consistent support.\nThe IPS program, funded by the City of Philadelphia Department of Human Services, is an after-school initiative providing early intervention services for youth exhibiting high-risk or at-risk behaviors.\nWhat You'll Do\nDirect Client & Family Support\nConduct initial assessments with clients and families.\nGather client/family information through interviews, case history, and observation.\nMake weekly home visits, often alongside the assigned Counselor.\nAssist with transportation of clients as needed.\nFamily & Community Engagement\nLink families to the program's parent education component and family activities.\nConsult with the IPS Counselor on referral needs - especially for substance abuse and mental health - and follow through on referrals.\nCollaborate with the IPS Counselor and team on step-down and discharge planning.\nCase Planning & Documentation\nCollaborate with the team to build each youth's Individual Service Plan.\nLead monthly case review meetings and track progress toward outcomes.\nDocument all casework, including assessments, IPS records, and monthly statistical/narrative reports.\nRepresent assigned youth at court appearances.\nParticipate in interdisciplinary meetings and weekly supervision.\nOther duties as assigned.\nRequirements\nQualifications\nRequired\nBachelor's degree in a relevant field (social work, behavioral health counseling, juvenile justice, education) -
