The Individual Service Coordinator (ISC) at YAI provides case coordination for the people we support as an advocate for our individuals between program, families, residences, and care \nmanagers. \nJob responsibilities\nManage a caseload of 50-70 PWS\n\nInput services and meeting minutes into Evero\n\nTaking notes in meetings\n\nAble to communicate with challenging families/residences\n\nUnderstanding of clinical needs of PWS\n\nLiaison between families/residences/care managers\n\nMaintain records in accordance with OPWDD regulations\n\nAssist with Audits\n\nEntering and Manage databases of PWS\n\nProvide coverage to the people we support base on programs needs\n\nEducation and Experience\nBA preferred\n\nExperience working with people with intellectual and developmental disabilities and advocating for their needs/desires.\n\nAdditional Knowledge, Skills and Abilities\nExceptional interpersonal, problem solving, organizational, Presentation and communication skills\n\nWilling to take initiative\n\nHighly flexible personality type\n\nAble to deal with challenging families/residences\n\nUnderstanding of person-centered philosophy\n\nKnowledge of Evero\n\nWork in Team Environment\n\nCompensation\n$19.05 / Hour\n\nAll qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, age, disability, or status as a Vietnam or disabled veteran.
