Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area.\nProgram Coordinator, Home Support Services (Part-Time)\nThe incumbent is responsible to the Program Manager/Assistant Director of Home Support Services, for the overall day to day coordination of Support Services to the community\nResponsibilities:\nCoordinates the delivery of appropriate home support services to enable clients to age in place\nCoordinates meal and transportation services for clients\nProvides timely and professional client intervention to clients and family\nMonitors overall health status and keeps up-to-date health records of clients\nCoaches, develops and evaluates staff to ensure they meet their required performance standard. Accurately schedules staff resources in accordance with clients' needs as reflected in the care plan\nEnsures implementation of risk management policy and safe environment for clients and staff\nIn consultation with the Manager, develops and updates policies, procedures for the program\nCoordinates the optimal deployment of volunteers for the program and provides effective guidance, training and support to the volunteers\nMaintains up to date and accurate files, records and statistics on the program\nEnsures efficient and responsible use of resources and maintains accurate record of fees and expenditures\nProvides emergency response to emergency calls from staff and clients\nEffectively promotes the home support services to clients of the building/neighbourhood\nKeeps up-to-date with developments and best practices in the field\nOther duties as assigned\nQualifications:\nBachelor's degree in social services, health care, activation, or equivalent\nMinimum 2 years experience in social services or health care, supervisory experience an asset\nKnowledge and experience in working with seniors\nComputer proficiency, with knowledge of assessment software, AlayaCare, and Microsoft Office Solutions\nCurrent CPR Certificate required\nSatisfactory Police Reference Check (with vulnerable sector screen) result\nLanguage Requirement: Ability to communicate in English and Chinese\nPhysical Demands/ Working Condition:\nOccasional travel\nUse of own vehicle\nPlease apply before:\n2026-10-24Salary Range:\n$34.28 - $40.32INTERNAL APPLICANTS: PLEASE APPLY THROUGH THE JOBS HUB\nOur people are our greatest strength and the core of our success.
