• Manageable Caseload: 3-4 patient assignments, which may include admits/discharges, per 12-hour shift

What You’ll Do

• Deliver acute, hospital-level care in patients’ homes

• Perform skilled clinical tasks (IV starts, phlebotomy, medication administration)

• Monitor patient conditions andrecognize changes requiring escalation

• Collaborate with avirtual and in-person interdisciplinary team

• Use evidence-based practice toensure safe, patient-centered care

• Coordinate discharge planning and ongoing care needs

• Promote aculture of safety and high-quality outcomes

What We’re Looking For:

• ActiveRN license in the state of Illinois

• BLS certification (AHA)

• Valid driver’s license with a clean driving record

• 2+ years of recent acute inpatient care or ED experience preferred

• Strong IV, phlebotomy, and clinical assessment skills

• Ability to workindependently in home settings

• Excellent communication and critical thinking

• Comfortable withtechnology and remote monitoring tools

Work Environment

• Travel within designated service area

• Provide care in private home settings

• Physical activity required (lifting, bending, standing, etc.)

• Exposure to clinical and environmental variables typical in-home care
