About us
Comcare is an established Mental Health and Housing provider assisting people with mental health or addiction issues to live well in the community. We provide a rich and diverse range of Mental Health Support Services, Housing and Housing Support Services.
We are looking for a Complex Case Coordinator to join our Community Integration Service.
The Community Integration Service works with people who have complex social or other issues that create barriers for successful community living and supports them to problem-solve a pathway forward to independence.
About this role
The successful person will have experience in the mental health arena and have skills in collaborative planning for clients with complex needs, thinking outside the square and developing service provision based on Te Whare Tapa Wha and strengths models.
What you’ll be doing
- Working closely with clients through a transition period between hospital/residential setting and more independent living.
- Implementing a transition plan for all involved workers which may include Social Workers, GP’s, Case Managers, MSD, family/whanau and friends. Clearly identify tasks for all, including the client and following up on tasks.
- Record keeping through a PMS system which includes plans and ongoing actions as per Comcare’s procedural requirements.
About you
To be successful in this role, you will have:
- Well-developed in the area of communication and interpersonal skills.
- A high level of literacy for planning and documentation.
- Problem solving/ solutions focused.
You must have a New Zealand Certificate in Health and Wellbeing Level 4 - Mental Health and Addiction Support (or equivalent qualification).
Experience in the mental health and disability sector would be an advantage.
Applicants must hold a current full, clean driver’s license.
You will have a cultural awareness, knowledge of, and demonstrable commitment to the principles of Te Tiriti o Waitangi.
Eligibility
