The Associate Director, Care Management provides operational leadership for multiple Care Management teams and leaders, with accountability for execution, performance, quality, productivity, and associate engagement. This role helps translate enterprise and segment priorities into consistent care management operations that support members, caregivers, associates, and business partners

The Associate Director demonstrates a strong understanding of how organizational capabilities, operational priorities, and care management strategies connect across departments. This leader translates those priorities into day-to-day execution by overseeing performance, workforce planning, productivity, process improvement, quality initiatives, and leader effectiveness across multiple teams

Key Responsibilities

The Associate Director, Care Management will:

  • Lead, coach, and develop care management leaders who are accountable for frontline care management associates, ensuring consistent expectations, strong leader routines, and effective support for team performance.
  • Foster a culture of engagement, accountability, inclusion, and continuous improvement that supports both associate and member experience.
  • Foster an environment of accountability, inclusion, engagement, and continuous improvement where leaders and associates understand expectations, use data to guide decisions, and stay focused on member and associate experience.
  • Monitor, analyze, and interpret performance data, trends, and outcomes to drive informed decision-making and operational improvements.
  • Achieve key performance indicators (KPIs) at both the individual and team levels.
  • Partner with Workforce Management and operational partners to evaluate staffing needs, inventory trends, productivity, scheduling impacts, and capacity planning across assigned teams.
  • Develop and implement initiatives that generate administrative savings while maintaining quality and compliance standards.
  • Maintain accountability for operational quality, audit readiness, compliance expectations, and consistent execution of care management processes across assigned areas.
  • Lead and support cross-functional projects and strategic initiatives from planning through execution.
  • Ensure alignment with organizational priorities related to quality, productivity, member satisfaction, and associate engagement.
  • Provide leadership and oversight for complex operational and performance challenges, identifying solutions that improve outcomes across multiple teams

Required Qualifications

  • Active Registered Nurse (RN) license in the state of Michigan.
  • Must reside in Michigan.
  • 6+ years of experience in clinical care, care management, or population health.
  • 2+ years of leadership experience managing leaders or care management professionals in a clinical, care management, managed care, or population health environment.
  • Demonstrated experience leading multiple projects and initiatives simultaneously from development through implementation.
  • Knowledge of Home and Community-Based Services (HCBS), Long-Term Services and Supports (LTSS), Medicaid regulations, and person-centered planning principles.
  • Proven ability to analyze, interpret, and present operational, quality, productivity, staffing, and performance data to senior leaders, managers, and frontline associates.

Preferred Qualifications

  • Advanced degree in nursing or a business-related field.
  • Progressive operational leadership experience in healthcare, care management, population health, or managed care.
  • Experience leading managers and/or supervisors within a multi-level leadership structure.
  • Experience with workforce planning, productivity management, and operational performance improvement initiatives.

Additional Information

  • Schedule/Time Zone: Monday through Friday, 8:00 AM - 5:00 PM Eastern Time zone with flexibility to work overtime as needed.
  • Work Location: State of Michigan
  • Work Style: Hybrid
  • Travel Requirement: Up to 25% travel within Michigan to attend market and leadership meetings, represent Care Management, and participate in onsite state meetings.

  • Number of Direct Reports: You will manage 1 to 9 direct reports

Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$104,000 - $143,000 per yearThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
