This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Director, Community Health Operations based in the United States.

This is a senior operational leadership role responsible for scaling community-based healthcare delivery across multiple markets and health plan partnerships. You will lead distributed frontline teams and ensure community health workers, nurses, and care coordinators operate as one integrated care team. The role combines workforce planning, operational performance, compliance, financial accountability, and care-model transformation. You will translate enrollment forecasts and care model requirements into practical deployment strategies across counties. You will also represent community operations in health plan discussions and ensure field execution matches contractual and regulatory expectations. This is an opportunity to shape a growing, technology-enabled care model while improving access and outcomes for underserved populations.

Accountabilities

    • Lead the operational performance of community health workers and Community Supports through experienced operational leaders, establishing clear goals, accountability, and performance standards.
    • Own geographic workforce planning, determining the roles, staffing levels, and hiring timelines required across counties based on enrollment forecasts and care model conversion schedules.
    • Integrate community-based teams with clinical care teams so community health workers, RNs, and care coordinators function as a unified care team rather than separate functions.
    • Maintain operational readiness for audits, health plan requirements, and Medicaid-related regulatory expectations, including DHCS readiness and fidelity requirements.
    • Own community operations performance reporting for health plan forums and provide clear, data-driven recommendations to internal and external stakeholders.
    • Partner with Finance to manage unit economics for Community Supports and the CHW Benefit, including evaluating the ongoing strategic and financial viability of the benefit.
    • Develop career pathways, training frameworks, management structures, and decision rights that enable a growing frontline workforce to operate effectively without excessive centralized oversight.
    • Establish deployment and performance measures that connect community health worker engagement to outcomes recognized by both health plans and financial stakeholders.
    • Lead operational transformation as the organization transitions to an RN-led care model across multiple health plan contracts and geographic markets.
    • Serve as the operational counterpart to Payer Operations for initiatives involving community-based care delivery.
    • Requirements

      • 8+ years of leadership experience in community health, care management, population health, or a related healthcare operations environment, including experience managing through other leaders.
      • Demonstrated success leading distributed frontline teams across multiple counties, markets, or geographic regions.
      • Strong Medicaid experience; direct CalAIM experience is strongly preferred, with Enhanced Care Management and Community Supports experience considered a significant advantage.
      • Proven ability to integrate community-based services into clinical care delivery and build workflows that support coordinated, whole-person care.
      • Strong operational and analytical skills, with the ability to use performance data to make decisions and confidently communicate or defend those decisions to health plan stakeholders.
      • Experience with healthcare audit preparation, regulatory readiness, compliance, and operational fidelity requirements in a Medicaid environment.
      • Demonstrated ability to design scalable workflows, organizational structures, career pathways, and management systems for growing frontline teams.
      • Strong leadership, communication, stakeholder management, and problem-solving skills, with the ability to work effectively across clinical, operational, financial, and payer-facing functions.
      • Experience in a technology-enabled healthcare delivery organization and/or during a period of significant operational transformation is preferred.
      • Experience with housing navigation, social services, or other programs delivered through Medicaid benefits is a plus.
      • Bilingual Spanish skills or experience leading teams serving predominantly Spanish-speaking populations is preferred.
      • An advanced degree in public health, social work, healthcare administration, or a related discipline is preferred.
      • Must be authorized to work in the United States; the position can be performed remotely from anywhere in the U.S.
