Legion is building autonomous medical care (the AI doctor), starting with psychiatry. Our AI-native care-delivery platform currently automates 95% of the administrative work required for us to deliver direct patient care. The role

Hey, I'm Daniel Wilson, co-founder and CTO at Legion. I studied applied math and CS at Princeton, led the Office Scripts and Power Automate integration at Microsoft, and did deep learning and RL research before that. GitHub: github.com/danielgwilson

I'm hiring a product engineer to ship the care loop: the path a real psychiatric patient travels through our platform. Booking a visit. The provider seeing their history at a glance. The prescription going out. The follow-up landing on the calendar. Right now our providers run thousands of visits a month on legacy tools while we move them onto the platform we built, and the engineer in this seat is the one who makes that platform good enough that nobody misses the old one.

This is not a ticket-taking job and it is not an infra job. It's the seat for someone whose instinct, handed "the provider can't scan the patient's meds fast enough during a live visit," is to go sit in on a visit, fix the screen, ship it that week, and then fix the three things they noticed while they were in there. The best engineer we've had in this lane got hired in 8 days and owned an entire e-prescribing integration within a month. That's the pattern this seat rewards.

What you'll own

  • The live clinic test. We're driving to a full clinic day on our platform with zero fallback to legacy tools. The current gap list is specific: prior-visit context at a glance, medication lists a clinician can scan in seconds, unambiguous pharmacy selection, prescription status you can trust, patient accounts and visit links that just work. You ship these.

  • The patient surface. Booking, intake, portal, messaging, reminders: the experience that decides whether a psychiatric patient in Texas feels taken care of or lost.

  • The provider surface. The screens a nurse practitioner uses with a live patient in the room. Scannability and correctness here are clinical safety, not polish.

  • The loop end to end. Visit to note to prescription to follow-up, as one coherent flow rather than four features. You'll work against our event-sourced clinical spine (orders, tasks, receipts) with a principal engineer owning the substrate under you.

  • Agent-assisted everything. We ship with AI agents as a matter of course, and your work goes to production weekly at minimum, usually faster.

What "a fit" looks like, concretely

  • You've shipped real product 0-to-1 or 1-to-N and owned it after launch: not tickets, features with your name on them and users who noticed.

  • You move across the stack without ceremony: Next.js and TypeScript on the front, Postgres and event-driven services behind it.

  • You use AI coding agents daily and can show us your setup. We care what you let them do and how you check them.

  • You have product taste under constraint: you can make a screen a stressed clinician reads in three seconds, and you know that's a different problem than making it pretty.

  • Handed an ambiguous feature, you go look at the real workflow (sit in the visit, read the transcript, call the ops person) before you write code.

  • Experience level: doesn't matter if you're cracked. Our best hire in this lane was early-career; the bar is slope and ownership, not years.

This role is not for you if you need a spec before you can start, you think of frontend as someone else's job (or backend as someone else's job), or shipping to real patients weekly sounds stressful rather than like the whole point.

Tech Stack

TypeScript and Node.js, Next.js 15 on Vercel, Postgres, event-sourced clinical records, production LLM agents (Anthropic, OpenAI) with tool-calling and evals, e-prescribing and EHR integrations, PHI-grade security and audit trails.

Process (7 to 10 days, we mean it)

  • Product deep dive (45 to 60 min). Walk me through something you shipped and owned: the decisions, the users, what you fixed after launch.

  • Hands-on technical (2 hours, in person or live). Build against a simplified slice of our real repo with your own agent tooling. The deliverable is working code, not a plan, plus your agent session logs.

    • [Alternate] Paid work trial (2 to 5 days, flexible around your current job). A real item from the clinic-test gap list, in the real repo, with your own agent tooling. It's the strongest signal for both sides, but most people can't take days off a job, so it's never required.

  • Decision within 48 hours. Written offer or a written no with the reason.
