We're early, with real patients across all 50 states and a founding team that's built, shipped, and scaled before. This is a founding-team hire: you'll own a function from day one and set the patterns the next ten people inherit. We hold a high bar - for precision, for follow-through, and for treating patients like people in command, not cases to manage.
The Role
You're the senior-most engineer setting how we build - and how we build is hybrid: you, the team, and the agents you direct. A genuine IC who still loves shipping code, but whose real leverage is the system around the code: specs agents can execute, verification harnesses that catch what line review misses, and guardrails that make speed safe. You own the outcome of every change that ships - typed by you or not.
What you'll do
Own and drive the technical platform: ship production code, set the architecture, and establish the patterns the rest of the team - including our agency partners - builds on and stays accountable to.
Run spec-driven development: decompose work into small, independently verifiable slices, delegate what agents do well, and keep architecture, security-sensitive paths, and core business logic in human hands - with a reasoned line for why.
Build the verification harness - tests, evals, CI guardrails - that reviews at the speed agents produce. When a bad change gets through, fix the harness that let it through, not just the change.
Shift quality and security left: treat agent-written code as unreviewed code, gate merges with blast-radius-calibrated review, and hold PHI/HIPAA as a design constraint from the first commit. In healthcare, this comes first - always.
Know when not to use an agent: deterministic code before reasoning loops, retire agentic components once the ambiguity is gone, and keep the codebase legible to the next engineer and the next agent.
What we're looking for
A strong IC coder where backend in Python, Go, or Rust is critical; full stack is ideal, including React/TypeScript. You've owned production systems end to end - proven, not theoretical.
DTC healthcare experience with hands-on PHI/HIPAA work: you've designed systems where compliance was a constraint, not an afterthought.
The judgment to set technical vision and architecture, and the temperament to hold a team to it without managing them.
Proven agentic practice: you've shipped real work through spec-driven delegation - and you can defend the line between what you hand to agents and what stays human. You know a 4,000-line agent PR is a process failure, and you can say why.
You've built the harness somewhere real: test suites, evals, or CI guardrails that caught what line review would have missed - and you own outcomes of code you didn't type.
Critical AI fluency: you direct agents daily, verify output instead of trusting polish, and notice when they're confidently wrong.
