Job Description
- Full-time Hospice Admission RN
- 5 days per week
- Territory: Counties surrounding Office Location
- Report to Clinical Coordinator and/or Intake Nurse each shift to learn of assigned visits/tasks for that day. Common assignments may include:
- Hospice evals or assessments
- Urgent or change of condition visits
- General visit coverage for a different RNCM when on PTO or when staffing demand is present
- Tuck-in visits
- GIP visits
- Other misc. PRN visits.
- Appropriate follow-up / coordination of care with the involved RNCM or other involved team members after completing a visit.

Requirements
- One year Hospice experience preferred
- 1 year of RN experience required
- Licensed Registered Nurse in State of Indiana
- Current driver's license
- Must have reliable transportation and insurance
