PrimaryMD is a concierge medical practice built around one idea: care that knows you, looks ahead, and follows through. Primary care, urgent care, and evidence-based longevity medicine come together in one membership, led by exceptional physicians who have the time to truly know their members.
Our members have more health information than ever: wearables, labs, biomarkers, imaging, specialist notes. What they need is clarity about what matters. We deliver it with great physicians and our own technology platform, which turns all of that information into one clear plan and makes sure every next step actually happens.
We are tripling our care capacity in under six months, with new markets to follow. We're hiring two Senior Product Engineers to own that platform end to end and help define what technology-enabled medicine looks like.

What you'll build
The operating system for our care teams. Our physicians, care coordinators, and medical assistants run their day on our platform: tasks, care plans, scheduling, and referrals. Make it the best clinical software they have ever used. Picture every coordinator starting the day with one brief that shows every member who needs something, and why.
The member experience. Between visits, members should feel their care team is right there with them. Build what they open every week: their results in context, their wearable data, their plan, and an easy way to reach the people who know them.
AI that carries real work. Agents that track every referral until the appointment is booked and the notes are back, triage the inbox, and draft documentation, so our team spends its time with members. Physicians stay in the loop: anything clinical ships with their sign-off.
Data that becomes clarity. EHR, labs, wearables, and biomarkers, connected, clean, and secure. Surface what deserves a physician's attention. When a member's sleep and resting heart rate shift for two weeks, their physician should know before the member thinks to mention it.
The whole loop. Talk to the people using it, decide what matters, design it, build it, ship it, and keep it running. Uptime, security, and patient privacy are yours as much as features are.
How we build
- You build with AI coding agents. They write most of the code; you own every line of it: architecture, review, testing, and what happens in production.
- You learn what to build by talking to the people who use it.
- What you ship this week gets used this week.
Who you'll work with
- Founder & CEO (your manager): sets product direction and builds alongside you day to day.
- The other Senior Product Engineer: shared ownership of the platform. You'll divide areas between you as we grow.
- Chief Medical Officer and Chief Science Officer: own clinical standards. Anything clinical you build goes through them.
- Our care teams in New York and San Francisco: your daily users and your fastest feedback loop.
What success looks like after year one
- Our care teams run entirely on the platform, with no side spreadsheets or workarounds
- Members open it every week because it helps them
- AI agents carry a real share of our coordination and admin work

What we're looking for
You must have:
- A track record of shipping and maintaining production software that real people use. Any background works. Your work is the credential.
- Full-stack range: data model, backend, frontend, integrations, and deployment. You can take an idea to production on your own.
- AI at the center of how you build. You work with coding agents (Claude Code, Codex, or similar) every day, you have a system for getting production-quality output from them, and you can show us.
- Strong product judgment. You figure out what to build by talking to users, and you care how it feels to use.
- Ownership. You see a problem, make the call, and ship without waiting for a spec.
- The ability to teach. We want to learn from you.
Bonus points:
- Experience in healthcare or health tech, or a real passion for it
- Experience with HIPAA, patient data, and healthcare systems (EHRs, labs, wearables)
- You've founded a company. If you've built and exited, and you miss seeing your work in real people's hands every day, this seat was made for you.

- Compensation: $175,000 to $240,000 base salary, based on experience
