About the Position

Under the direction of the Quality Director/ Patient Safety Officer, the Patient Safety Specialist serves as a facilitator in developing, implementing, analyzing, coordinating, and evaluating patient safety events, systems, and processes. Coordinates performance improvement and patient safety initiatives to support a culture of safety and reduce health care errors and risks. This may include participation in data analysis, chart reviews, root cause analysis, focus studies specific to patient safety, staff/patient education, patient safety rounding and observations. The Patient Safety Specialist will function as an expert and educator on relevant patient safety initiatives and will lead or participate in multidisciplinary projects and improvement teams.

Overview of Responsibilities

· Reviews and edits all patient safety events to ensure completeness, accuracy, and timeliness of all events are reported to key stakeholders.

· Analyzes and coordinates patient safety activities and initiatives.

· Works to encourage and support a culture of safety in the organization.

· Investigates and evaluates events for risk potential, loss minimization, mitigation, and proper documentation.

· Assists with the coordination and communication of patient safety initiatives.

· Provides consultation and support to teams and departments related to performance improvement initiatives and patient safety events.

· Assists in managing data, identifying trends, preparing graphs and power point slides.

· Collaborates professionally with staff, managers, and providers in addressing and resolving patient safety issues.

· Facilitates event review process, and root cause analysis (RCA), failure mode and effects analysis (FMEA), and cluster investigations, including leading the team to consensus on scoring and identifying gaps in safety events.

· Utilizes performance enhancing tools as part of process improvement, problem analysis, and measurement to lead projects.

· Effectively uses data to communicate, including the ability to present data

Education & Experience

  • Bachelor’s degree in nursing

  • Three years’ experience in acute care hospital required.

Licensure & Certification

  • Minimum licensure required: Licensure in State of Indiana as a Registered Nurse
