Role Overview

As the Home+ Community Case Manager, you will provide trauma-informed, survivor-centered case management to survivors of domestic and gender-based violence while building community partnerships and expanding access to resources. You will maintain an assigned caseload and provide individualized advocacy and support that promote housing stability, economic empowerment, safety, and well-being.

Through outreach, presentations, networking, and collaboration with community organizations and service providers, you will strengthen referral pathways and awareness of Home+ services, particularly in communities facing systemic barriers to care. This role calls for compassionate advocacy, a commitment to social justice and anti-oppressive, healing-centered engagement, and the ability to work independently in the community and collaboratively with the Home+ team.

This position is based in both Manhattan/Harlem and Queens, with work at Rising Ground’s Harlem and Queens hubs, the Manhattan and Queens Family Justice Centers, and community locations in both boroughs.

Key Responsibilities

  • Provide trauma-informed, survivor-centered case management to an assigned caseload, including intake assessments, individualized service and safety planning, advocacy, and ongoing support.
  • Help survivors navigate housing challenges and access public benefits, financial assistance, flexible funding, legal services, and resources that support safety and economic empowerment.
  • Develop and implement outreach activities that increase awareness of Home+ services, expand the program’s reach, and connect with underserved communities.
  • Conduct outreach visits and deliver presentations to community organizations, social service agencies, healthcare providers, schools, faith-based groups, and other stakeholders on Home+ services, eligibility, referrals, and survivor supports.
  • Research and connect with resources for housing, food security, employment, education, childcare, transportation, healthcare, legal assistance, and emergency financial support.
  • Build relationships with community partners, government agencies, grassroots organizations, and service providers; coordinate communication and referrals to reduce barriers, address service gaps, and improve access to care.
  • Identify opportunities to secure community resources, donated goods, emergency assistance, and other services that meet survivors’ immediate and ongoing needs.
  • Represent Home+ at community meetings, resource fairs, networking events, coalitions, and collaborative initiatives to promote services and strengthen partnerships.
  • Maintain an updated community resource directory with services, eligibility criteria, referral procedures, and contact details, and share relevant resources with the Home+ team.
  • Collaborate with the Home+ Supervisor, Case Managers, and other Rising Ground programs to identify outreach priorities, emerging community needs, and opportunities to improve survivor access.
  • Maintain accurate, timely, confidential records of case management, outreach, presentations, partnerships, referrals, and identified resources in accordance with agency policies and program requirements.
  • Participate in supervision, team meetings, case conferences, professional development, and quality improvement efforts that strengthen community engagement, accessibility, and survivor-centered services.

Perform other related duties as assigned by the supervisor. This description does not cover every activity, duty, or responsibility required for this position. Duties, responsibilities, and activities may change at any time, with or without notice.

  • Experience working with diverse populations, including BIPOC, LGBTQ+, immigrant, and justice-involved communities.
  • Strong knowledge of New York City housing systems, public benefits, emergency financial assistance programs, and legal advocacy resources.
  • Commitment to anti-oppressive, harm reduction, and restorative justice approaches.
  • Ability to work independently and collaboratively within a team and across disciplines.
  • Excellent organizational, communication, and crisis intervention skills.
