Position Summary:

The ACT Team Leader/Supervisor serves as the clinical and administrative head of the Assertive

Community Treatment (ACT) team. This position requires a minimum of an independently

licensed Master’s level qualified practitioner or a Master’s level Registered Nurse (RN). The

Team Leader/Supervisor oversees the day-to-day operations of the ACT team, ensuring

adherence to clinical protocols, regulatory requirements, and quality standards. They provide

clinical supervision, guidance, and support to team members, including clinicians, case

managers, and peer support specialists. Responsibilities also include program development, staff

training, and collaboration with community partners to enhance client care and outcomes. The

Team Leader/Supervisor reports to senior management and contributes to strategic planning

initiatives aimed at improving service delivery and client satisfaction within the ACT program.

ACT is an intensive, integrated, rehabilitative, crisis, treatment, and mental health community

support service provided by an interdisciplinary team to individuals eighteen (18) and over with

serious and persistent mental illness with dedicated staff time and specific staff-to-consumer

ratios.

Service coverage by the ACT team is required twenty-four (24) hours per day, seven (7) days per

week. Provides co-occurring disorders treatment using evidence-based and stage-appropriate

interventions.

Key Responsibilities:

 Provide clinical supervision and oversight to the ACT team.

 Ensure compliance with regulatory standards and best practices in mental health services.

 Develop and implement treatment plans in collaboration with team members.

 Monitor and evaluate team performance, ensuring quality service delivery.

 Facilitate staff meetings and training sessions to enhance team competencies.

 Collaborate with external stakeholders, including community agencies and healthcare

providers.

 Participate in program evaluation and quality improvement activities.

 Maintain accurate documentation and records related to client care and team operations.

 Support crisis intervention and management as needed.

 Uphold ethical standards and promote a culture of professional excellence within the

team.

Qualifications:

 Master’s degree in Social Work, Psychology, Counseling, Nursing, or related field.

 Current state licensure as an independently licensed practitioner (LCSW, LMFT, LPC,

etc.) or Master’s level RN.

 Minimum of [X] years of experience in mental health or related field, with [Y] years in a

supervisory capacity preferred.

 Strong leadership, communication, and organizational skills.

 Knowledge of ACT principles and evidence-based practices in mental health.
