Join Our Team as a Community-Based Care Surveyor!

The Aging and People with Disabilities (APD) program is seeking a passionate individual to join our Community-Based Care (CBC) team in Tigard, Oregon. We have an opening for a Community-Based Care Surveyor (Client Care Surveyor) who is committed to making a meaningful impact in the lives of Oregon’s older adults and people with disabilities.

As a CBC Surveyor, you’ll play a critical role in ensuring that individuals and their families receive person-centered services that promote safety, independence, and well-being-while honoring choice, respecting cultural preferences, and upholding dignity. If you’re driven by innovation, early intervention, and the desire to make a difference in your community, this is your opportunity to help advance the mission of the Office of Aging and People with Disabilities.

Summary of Duties

As a Community Based Care Surveyor, you will:

  • Conduct state licensing surveys according to state protocols, guidelines, and time frames.

  • Review facility survey history, complaint investigation reports, and data reports.

  • Observe details of residents’ quality of care and life, including staff assistance with activities of daily living, staff interactions with residents, nutrition, activities provided, and environment.

  • Interview residents, families and facility staff.

  • Review resident medical records and facility policies and procedures.

  • Evaluate conditions, using OARs, state guidelines, and individual professional judgment to validate and determine findings of potential non-compliance.

  • Perform on-site inspection of the long-term care facility, including an inspection of the kitchen and other areas where food is prepared for residents.

  • Write report of findings, using specified computer software program.

Minimum Qualifications

  • Three years of experience in areas related to long-term care or nursing home administration AND One of the following:

  • A bachelor's degree of Health Science, Gerontology, Healthcare Administration, Long Term Care Administration

  • OR

  • A license as a Registered Nurse

  • OR

  • A license as a Long-Term Care Facility Administrator with a Bachelor’s in health services

  • Note: LPN certification and/or experience will not meet the minimum qualifications.

  • A valid driver’s license and acceptable driving record is required for this position.

Essential Attributes

We are looking for candidates with:

  • Experience related to nursing in a long-term care setting or administration of a nursing care facility or assisted living.

  • Long Term Care Rules/Regulations: Knowledge of Oregon State laws, rules, and regulations related to the provision of long-term direct-care programs and facilities.

  • Critical Thinking, investigation, and decision making: Skill in exercising independent judgment and decision-making in the application of laws, rules, policies, or procedures.

  • Teamwork, self-awareness, and growth mindset: Demonstrated ability to work successfully both independently and cohesively with a team.

  • Demonstrates emotional intelligence, conflict resolution skills, and unwavering professionalism-even in high-pressure or adversarial situations.

  • Experience using a variety of Microsoft Office Programs, email, data entry, video conferencing, and instant messaging.

  • Skilled in technical writing using the Principles of Documentation, with experience creating clear, accurate, and objective reports for medical, clinical, or long-term care investigations.

Working Conditions

  • This role involves both community-based work and in-office, with very limited opportunities to work from home. Your primary office location will be in Tigard. Training and trial service will take place in Tigard, Oregon, for the first nine months.

  • Travel regularly, often overnight, Monday through Friday to counties across Oregon, for up to two weeks each month.

  • Work daily within busy and varying long-term care healthcare facility settings.

  • Lift and carry up to 20 pounds throughout each day.

  • Irregular work schedules and frequent schedule changes.

  • You may interact with a wide range of people including some who have experienced trauma and exhibit challenges in controlling their emotions.

  • Walking into stressful situations that may require quick decision-making skills to ensure safety of those being serviced and own physical safety. This may also be trauma inducing.

  • You will be a mandatory reporter of abuse and neglect of children, older adults, and other adults with disabilities.

  • The Oregon Department of Human Services does not offer visa sponsorship. Within three days of hire, you will be required to complete the US Department of Homeland Security’s I-9 form confirming authorization to work in the United States.
