There is an exciting opportunity to join CAMH’s Toronto Mental Health and Addictions Coordinated
Access team! CAMH is currently seeking (6) full-time, Permanent Service Coordinator to provide client-centered
intake, screening, and service coordination across the mental health and addictions (MHA)
system.
Centre for Addiction and Mental Health (CAMH) has been selected as the Mental Health and Addictions
Hub to establish a centralized access point for outpatient hospital and community mental health and
addictions service in the Toronto Region. This Hub is responsible for coordinating timely access to
services and will play a key role in transforming access to care.
Facilitate access to care by reviewing referrals, engaging with clients and families, conducting
brief screening, and connecting clients to appropriate services.
Review incoming referrals and determine eligibility for MHA services, triaging referrals to
appropriate priority levels and service type and matching to the most appropriate service
provider utilizing various referral management system.
Contribute to process improvement, stakeholder engagement and data collection to ensure the
provincial model is implemented with efficiency and equity.
Liaise with individuals, referral sources and service providers to ensure timely and appropriate
access to care.
Participate in regional exchange sessions and quality improvement activities to enhance service
delivery.
Foster positive relationships with referral sources, community partners, and service providers
across the Toronto Region
Maintain confidentiality of patient information in alignment with professional and
organizational requirements

This position is located at 1051 Queen Street West. The position will require Monday to Friday
day and evening shifts (0800-2000).
Bachelor’s degree in Social Work, Psychology, Health Sciences, Behavioural Sciences, Nursing,
or, related field is required
A Regulated Healthcare Professional (Social Work, Occupational Therapy, Nursing) is preferred.
Must be a member in good standing with the respective regulatory body.
3-5 years’ experience in the mental health and/or addictions sector providing direct patient/client care, including intake, service navigation, or case management.
Experience in coordinated access for mental health and addiction services preferred
Ability to efficiently synthesize information from a variety of sources (referral form, patient and
family interview, Connecting Ontario, previous medical records) to determine appropriate care
need and match individuals to the right care.
Good clinical judgement to identify urgency of care need and respond using established
protocols (eg. 911 calls, transfer to crisis service).
Excellent organizational, interpersonal, and written communication skills and the ability to work
effectively in a fast-paced clinical environment are essential.
Proficiency in Microsoft office, Hospital Medical Record Systems and electronic communication
tools required
Familiarity with referral management system an asset
Work effectively as a team member in a dynamic interprofessional environment and be a selfstarter.
Experience working with clients, staff, and organizations with diverse backgrounds is required.
Bilingualism (French/English) or proficiency in a second language is an asset.

Salary Range: $35.38 - $47.69 per hour
