  • Lead process improvement, future-state design, and operating-model transformation for Duals Care Management and D-SNP priorities across market and enterprise clinical operations.
  • Apply Lean, Six Sigma, root cause analysis, Kaizen, value stream mapping, structured problem solving, and current-state/future-state design to identify operational problems, reduce unnecessary variation, and develop sustainable solutions.
  • Assess differences across markets, health plans, products, platforms, and Care Management delivery models, and help determine what should be enterprise-standard, product- or platform-configurable, or market-owned.
  • Conduct process observations, analyze operational data, synthesize findings, and facilitate working sessions that drive decisions and actionable solutions.
  • Design scalable future-state workflows, capabilities, roles, handoffs, decision rights, controls, governance, and performance measures that enable D-SNP Care Management strategies.
  • Translate future-state operating models into clear business, workflow, data, and platform capability requirements in partnership with technology and operational teams.
  • Convert strategic, regulatory, contractual, product, and market requirements into practical Care Management processes.
  • Partner across Duals Care Management, market clinical operations, Medicare and Medicaid product teams, Behavioral Health, Pharmacy, quality, technology, data and analytics, compliance, community resources and member-support functions, and other enterprise teams.
  • Help scale successful market- or platform-specific practices into repeatable enterprise capabilities while preserving required market, product, platform, and regulatory flexibility.
  • Define operational, quality, member-experience, adoption, financial, and sustainability measures; establish monitoring and control mechanisms; and validate sustained improvement after implementations
  • Serve as a hands-on transformation partner to frontline teams, market leaders, business owners, clinical leaders, and executives, not primarily as project or program support.

Education/Experience: Bachelor's degree in related field or equivalent experience. MBA or MHA preferred. 6+ years of LEAN, Six Sigma or related process improvement experience, preferably in the healthcare industry. Previous working knowledge of process mapping and design, statistical applications and project management software.

Preferred Experience:

  • Strong process improvement and operating-model design experience in complex, matrixed healthcare environments, ideally Medicare and Medicaid Duals, D-SNP Care Management, integrated care, market operations, healthcare quality, regulatory, or medical-cost transformation.
  • Prior Duals or D-SNP experience is strongly preferred but not required for candidates with strong healthcare process improvement, learning agility, and operating-model design experience.
  • Demonstrated experience evaluating variation across markets, platforms, products, health plans, or business units; distinguishing required local or product-specific variation from unnecessary inconsistency; and translating findings into scalable operating models.
  • Experience designing future-state capabilities that require coordination across clinical operations, product, technology, data, quality, and market teams.
  • Strong capability in current-state assessment, process mapping, root cause analysis, Kaizen and improvement-event design, value stream mapping, future-state design, workflow redesign, stakeholder facilitation, adoption planning, measurable benefit validation, and structured problem solving.
  • Ability to influence without direct authority in complex, matrixed organizations.
  • Strong business acumen, analytical thinking, organization, and executive-ready communication skills. Licenses/Certifications: LEAN/Six Sigma Black Belt or Master Black Belt certification preferred.
Pay Range: $87,700.00 - $157,800.00 per year
