This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Clinical Services Coordinator based in the United States.

As a Clinical Services Coordinator, you will play a central role in connecting patients with the care, services, and resources they need.

You will support patient outreach, scheduling, care coordination, preventive care, and transitions of care across diverse populations.

Working closely with interdisciplinary teams, you will help improve access, close care gaps, and strengthen continuity of care.

You will use electronic medical records, population health data, AI-enabled tools, and reporting platforms to identify opportunities for intervention.

The role also contributes to quality improvement, workflow optimization, and the development of technology-enabled clinical services.

You will work with patients, caregivers, providers, community organizations, and care teams in a collaborative, mission-driven environment.

This fully remote opportunity is ideal for an experienced care coordination professional who is bilingual in English and Spanish and passionate about improving health outcomes.

Accountabilities

    • Conduct proactive patient outreach to support Annual Wellness Visits, transitions of care, preventive screenings, chronic disease management, care management, and other clinical initiatives.
    • Schedule, coordinate, and confirm patient appointments while identifying and addressing barriers that may affect access or adherence.
    • Coordinate services across primary care, specialty care, behavioral health, and community-based organizations.
    • Build trusted relationships with patients, caregivers, providers, and community partners to improve access and outcomes.
    • Educate patients, reinforce care plans, and support self-management and engagement in their care.
    • Help patients navigate healthcare services, insurance resources, transportation options, and relevant community resources.
    • Use electronic medical records, population health registries, AI-enabled tools, and reporting platforms to identify patients requiring preventive services, follow-up, or quality interventions.
    • Support preventive care programs, Annual Wellness Visit workflows, quality measures, and value-based care initiatives.
    • Assist with standardized assessments and screenings, including health risk assessments, PHQ-9s, and social determinants of health screenings when appropriate.
    • Monitor assigned patient work queues and outreach lists to ensure clinical and operational priorities are completed on time.
    • Conduct timely outreach following emergency department visits, hospitalizations, and other care transitions.
    • Coordinate post-discharge appointments, medication reconciliation or review, referrals, and follow-up activities as directed.
    • Identify and escalate clinical, behavioral health, or social concerns to licensed clinicians or appropriate care team members.
    • Collaborate with providers and interdisciplinary teams to improve patient engagement, reduce care gaps, and achieve population health and quality goals.
    • Participate in quality improvement initiatives, workflow optimization, and implementation of new clinical programs.
    • Help establish, document, and maintain standardized workflows, best practices, and clinical service processes.
    • Support onboarding, training, and knowledge sharing with team members as needed.
    • Contribute to technology-enabled patient engagement initiatives, including AI-assisted outreach tools and other digital solutions.
    • Maintain complete, accurate, and timely documentation within electronic medical records and care management platforms.
    • Follow organizational policies, regulatory requirements, privacy standards, and documentation guidelines.
    • Monitor outreach activities and documentation to support performance reporting and quality metrics.
    • Collaborate with internal teams to identify challenges affecting health centers and contribute to practical solutions.
    • Requirements

      • At least 3 years of experience in care coordination, care or case management, patient outreach, or connecting patients with community resources.
      • Bilingual English and Spanish communication skills, with fluency in medical terminology.
      • Experience working in one or more of the following settings: safety-net or public health hospitals, Federally Qualified Health Centers (FQHCs), academic medical centers, ambulatory care, physician groups or professional practices, community mental health centers, substance use treatment settings, or outpatient mental health services.
      • Experience engaging patients with chronic illnesses and complex behavioral health needs.
      • Understanding of the social factors and barriers that can influence health outcomes and access to care.
      • Experience supporting patients with multiple or complex social needs.
      • Knowledge of Medicaid and Medicare populations is desirable.
      • Familiarity with value-based care models and population health programs is desirable.
      • Strong patient engagement, communication, and relationship-building skills.
      • Excellent organization, time management, follow-up, and documentation abilities.
      • Ability to work effectively with interdisciplinary teams and diverse stakeholders.
      • Strong judgment and ability to identify concerns that require escalation to licensed clinical professionals.
      • Comfortable using electronic medical records, care management platforms, reporting tools, and technology-enabled workflows.
      • Ability to adapt to evolving processes and contribute to technology and AI-enabled clinical initiatives.
      • Strong commitment to patient-centered care, confidentiality, accuracy, and service quality.
      • Ability to work independently while remaining collaborative and accountable in a fully remote environment.
      • Availability to work during standard business hours of 8:00 AM-5:00 PM Central Time (CT).
