POSITION SUMMARY

The Admissions Coordinator is responsible for supporting admissions to the Jewish Home & Rehab Center. The position works closely with acute-care hospitals and community stakeholders to ensure a safe transition between care settings for new residents, acting as a liaison between the Admissions team, the Community we serve, and the Home’s Interdisciplinary teams.

The Admissions Coordinator will work closely with the Director of Admissions, playing an integral role in helping to achieve the organization’s census goals, growth, and revenue objectives.

KEY RESPONSIBILITIES

  • Assist in the management of LTC census and patient/resident ‘throughput’ with a focus on meeting/exceeding stated census goals and targets.
  • Support the management of application throughput and data entry via different programs/modalities (EMR, Allscripts, Curaspan, Referral Manager PRO, Fax, Phone, Email, and LTC roster).
    • Admitting short-term to LTC and LTC application process before patient arrives from hospital.
  • Monitor and provide ongoing phone, email, and customer service support to the Admissions office for both Short-Term Care and Long-Term Care inquiries. Provide tours of units. Respond to inquiries and applicants in a timely manner.
  • Communicate with Case Managers regarding the admission process and updates to ensure a smooth transition from Hospital care to a skilled nursing setting.
  • Assess and review Long-Term Care applications for completion and appropriateness for potential admission. Communicate with applicants for follow-up until application is complete and determination for admission and/ or admission has been finalized.
    • Works with “patient financial counselor” to complete financial review, insurance eligibility, and “assist” with Medi-Cal and SSI communication to prevent delays in admission.
    • Works with social services/TCCs and LTC clinical team to process and assess clinicals and roommate compatibility.
    • Manage waitlist
    • Manage and communicate move-ins to LTC quickly to avoid long-term vacancies.
  • Perform room check to ensure room is prepared for admission & order DME as needed.
  • Work with Clinical Liaisons to identify and follow up on clinical needs/requirements for admission and communicate to Interdisciplinary team as appropriate.
  • Create and complete admissions paperwork for incoming admissions, input incoming admissions into EMR, take patient photograph, upload photo & appropriate documents to EMR.
  • Look up, review, and verify insurance benefits/eligibility for potential patients/residents. Obtain authorizations as needed. Demonstrate understanding of contracted insurance providers, correctly input payer information into EMR, verify and save verification documents into Admissions drive.
  • Establish and maintain significant professional relationships with referral sources, physicians, and other healthcare professionals to increase referrals.
  • Represent JHRC in a professional manner in the community. Demonstrate patience, positive disposition, and enthusiasm with experience in customer service - “forward-facing” expectations, roles and responsibilities.
  • Attend staff meetings and educational in-services as scheduled.
  • Perform other duties as assigned (as they relate to the Admissions department). This includes cross-training and coverage for other positions within the department.

In addition, may perform one or more of the following:

  • Create admissions paperwork for all incoming admissions. This includes relevant payer information (patient responsibility form), as well as general information about the Home.
  • Create name tags and information sheets for incoming admissions. Information sheets include Care Team contact information and Care Conference schedule.
  • Running necessary screenings for potential admissions (i.e, Megan’s Law, Immunizations, PASSR, etc)
  • Complete admissions paperwork, uploading appropriate documents to EMR.
  • Prepare and pass Admissions Memo to respective nursing units
  • Welcome newly admitted patients and assist them to their room; familiarize patients with the call light, TV channels, phone, fan, and lighting.
  • Provide patients/families with general customer service (when needed).
  • Take patient photographs and upload them to EMR.
  • Provide tours of the units for potential patients/families.
  • Assist the team to answer call lights in a timely fashion and assist patients in getting needs met.
  • Monitor cleanliness of both F-building elevators and entryway floor. Report cleaning requests to housekeeping & repair requests to maintenance.
  • Perform other duties as assigned.

JOB DETAILS

  • Pay Rate: $35 - $40 per hour
  • Hours: Full-Time, 40 hours per week
  • Shift Schedule: Day
  • Days Off: Saturday & Sunday
  • Working Days: Monday through Friday

REQUIRED QUALIFICATIONS

  • High school diploma or equivalent, required. AA/AS/BS/BA, preferred.
  • Skilled nursing billing & insurance experience, preferred
  • Demonstrated proficiency with MS Office, including Outlook, Word, and Excel, required.
  • Demonstrated experience working in a fast-paced environment, required.
  • Ability to multi-task with a sense of urgency while maintaining a sense of sensitivity, required.
  • Demonstrated high level of interpersonal skills to handle sensitive and confidential situations.
  • Demonstrated ability to develop and build strong, lasting relationships with referral sources, clients, colleagues, and families.
  • Excellent organization, customer service, and communication skills, both written and verbal.

PHYSICAL REQUIREMENTS

  • Ability to lift, push, pull a minimum of twenty-five (25) pounds.
  • Ability to exert maximum muscle force to lift, push, pull or carry
  • Ability to use abdominal and lower back muscles over time without fatigue
  • Ability to stand and/or sit for extended periods
  • Ability to bend, stoop, stretch, twist, sit, and reach
  • Fine motor skills
  • Good visual and auditory acuity
