CareMessage is the technology non-profit building the largest patient engagement platform for low-income populations in the United States. Powered by the Health Equity EngineTM, the platform enables organizations to combine messaging, data, and interoperability to increase access to care, improve clinical outcomes, and address social drivers of health. With 20 million patients reached since 2013, CareMessage is the only patient engagement solution proven to improve health equity at scale. The team, many with lived experiences in these communities, leverages a nonprofit model to reinvest revenue into impact. CareMessage is the partner of choice for organizations committed to advancing health equity.

About CareMessage

CareMessage is a social enterprise and the largest patient engagement platform for low-income populations in the United States. We deliver personalized health education and care coordination through text messaging - reaching patients where they already are, in the language they speak, at the moments that matter most. Unlike technology built for large health systems and adapted down, CareMessage is designed from the ground up for organizations serving low-income, historically marginalized communities.

Why Join Us in Leveraging Technology to Improve Health Equity

Many of us have had the experience of doing good work or building great products, but wondering if it is truly making an impact. At CareMessage, our mission is clear: improve health equity for low-income populations across the United States. Every role here connects directly to that purpose. We care deeply about the communities we serve, and we also hold a high standard for performance, collaboration, and integrity.

Our ideal team members:

  • Deliver meaningful, measurable results

  • Invest in their teammates and build trust

  • Seek feedback and continually grow

  • Stay steady and solutions-oriented in ambiguity

  • Lead with integrity and responsibility

  • Communicate with clarity and empathy

If you are motivated by impact, energized by collaboration, and ready to do some of the most meaningful work of your career, we encourage you to apply. If this sounds like you, we would love to meet you.

About the Role

The Engineering Manager, Data & Clinical Integrations owns and drives the technical direction of the two teams responsible for CareMessage's clinical data. The Clinical Integrations team connects CareMessage to electronic health records (EHRs) and other clinical data sources such as HIEs and payor data across our customer base. The Data team turns what those integrations deliver into trustworthy insight. CareMessage supports over 400 safety-net healthcare organizations, including federally qualified health centers and free clinics that care for low-income and underserved patients nationwide, and these two teams are the engine behind our ability to measurably demonstrate impact on clinical outcomes, access to care, and the social drivers of health.

This is a leadership role over existing teams. Their systems and roadmaps are already in place, and your job is to lead, deepen, and scale them. At the core of that work is leading both teams to turn clinical data from our customers' many different EHRs into trustworthy, query-ready data that can measurably demonstrate health outcomes, then scaling those insights through a combination of AI/ML and human analysis.

The Clinical Integrations team builds and maintains the integrations that connect CareMessage to leading EHRs (Epic, eClinicalWorks, NextGen, athenahealth, and others) over HL7, FHIR, proprietary APIs, and direct-database interfaces using the Rhapsody integration engine. It owns the first step of the clinical data pipeline: converting clinical data to FHIR and landing it in our FHIR Store and BigQuery. The Data team builds on top of that with a medallion (bronze/silver/gold) architecture in BigQuery using native Google Cloud services and dbt, surfacing insight through Metabase dashboards such as our Health Equity and diabetes-outcomes views, and increasingly powering AI/ML workflows that surface insight at scale. In this role you will work closely with Product, Customer Success, Implementation, Compliance, and the rest of Engineering to design and deliver the platform and integrations that deepen patient engagement and health outcomes, with direct ownership of technical decisions and customer-facing system behavior.

Our ideal candidate is a technical, people, and process leader alike, who thrives in a fully distributed environment and is energized by the intersection of healthcare interoperability and data. They are dedicated to the professional growth of engineers, and they see themselves as a catalyst for organizational outcomes rather than a coordinator of tasks. They take a scientific approach to the work, relying on evidence alongside intuition and looking for data that challenges their assumptions before drawing conclusions. Above all, they take pride in advancing health equity for low-income populations.

What You Will Do

A. Lead and develop two engineering teams to integrate clinical data and systems that help us data-driven interventions and measure impact at scale

  • Own hiring, feedback, performance, and career growth for the engineers across the Data and Clinical Integrations teams

  • Set the operating cadence for both teams, including planning, retrospectives, and tech debt reviews

  • Set the performance bar for both teams and hold it, with specific feedback in the moment, evidence documented as you go, and decisions that are fair to the person and to the team

  • Run hiring for your own teams end to end, because managers own their own pipelines here

  • Proactively identify gaps in team capability, process, or priorities, and act on them without being asked

B. Own the data pipelines that build a comprehensive view of every patient so that we may deliver targeted interventions and assess outcome improvements

  • Hold the technical direction from EHR integration through to the modelled layers and dashboards

  • Manage the shared roadmap and dependencies between the two teams so neither is blocked waiting on the other

  • Set responsible-use guardrails for AI-assisted engineering across both teams, with particular care around PHI

  • Stay close enough to the systems to answer a question in SQL, review a technical spec, or prototype an idea

  • Be familiar with the “why” behind the products you support, and push back when team output doesn't reflect health equity principles

C. Turn clinical and non-clinical data into demonstrated health outcomes and health equity impact

  • Connect the team's work to our mission, ensuring our organization continues to evolve our impact evidence

  • Partner with Product, Customer Success, Implementation, and Compliance to define what we need to prove and what data makes it provable

  • Scale insight generation by combining AI/ML with human analysis, without loosening the standard for what counts as trustworthy

  • Own the accuracy and defensibility of the data that reaches our customers, our board, and our funders

What You Will Achieve

Within 30 days: Take over the teams and learn the pipeline end to end, with a focus on understanding how we go from integrating a system to reporting an outcome improvement

  • Own 1:1s and the operating cadence for both teams

  • Have a working understanding of the pipeline end to end, across clinical integrations, data transformation, the medallion architecture, and reporting

  • Deliver a written triage decision on the full open Data backlog: what we do, what we defer, and what we drop

  • Have a working relationship with your counterparts in Product, Customer Success, Implementation, Sales, and Compliance

Within 60 days: Establish clear ownership and a shared roadmap

  • Own the delivery commitments for our clinical data coverage targets

  • Have a named owner for every system and pipeline in your scope, with nothing left unowned

  • Have a shared roadmap across the two teams, with dependencies made explicit rather than discovered

Within 90 days: Run both teams independently and raise the bar for Engineering

  • Run both teams' cadence without the Director in the room

  • Have given every engineer at least one documented feedback conversation and a named growth goal

  • Have shipped one improvement that changes how all of Engineering operates

Key Performance Indicators (KPIs) for this role may include:

  • Clinical and non-clinical data coverage: growth in patients with intervention-ready clinical data and availability of expanded data for social drivers of health

  • Pipeline reliability: integration and pipeline failures found by us before a customer reports them

  • Backlog health: no unassigned work older than 90 days

What You Will Bring

  • You have managed engineering teams through a full performance cycle. That means hiring, consistent feedback, growing people when no promotion is available, and acting when someone is not meeting the bar for their level.

  • You have owned a production data or systems-integration platform end to end, from ingestion through to the modelled layer people query, in an environment with regulated or sensitive data.

  • You have led delivery across two or more teams with a hard dependency between them, and sequenced the work so neither team stalls waiting on the other.

  • You stay hands-on enough to be useful. You can answer a question in SQL, dig into the data tooling, and unblock the team without taking the work over.

  • You have brought AI-assisted engineering practices onto a team, coached people to use them, and set responsible-use guardrails where sensitive data is involved.

  • You are comfortable with health equity work. The decisions you make here determine what we can prove about outcomes for underserved patients, and you will be expected to apply that lens to your team's priorities, not just endorse it.

Location

This is a remote role within the United States. Occasional travel is expected for team offsites and company gatherings.

As a healthcare technology company, we love innovation, and AI can be a great assistant but you are the main event in this interview process. While we recognize AI as a valuable tool and encourage thoughtful use, over-reliance on it may not truly reflect your unique strengths. Our interview process is designed to assess individual skills, critical thinking, and problem-solving abilities. We encourage candidates to ensure their work reflects their own experiences and approach. We’re excited to learn more about you!
