INFUSION CARE SPECIALIST (LVN)

The Infusion Care Specialist serves as a dedicated clinical and operational partner for members receiving infusion therapies. This role supports members, providers, infusion centers, specialty pharmacies, and health plans by facilitating benefit navigation, treatment coordination, member education, and timely access to medically necessary infusion services. The Infusion Care Specialist helps remove barriers to care while ensuring a positive member experience throughout the infusion treatment journey.

Location:  Hybrid position. Candidates must be Dallas area based, working from home and/or in-house as necessitated by the business. Remote days require a private, secure home workspace free from other employment, caregiving, or school obligations during working hours.

Responsibilities:

• Manage an active caseload of members receiving infusion therapies across oncology, immunology, gastroenterology, neurology, rheumatology, and other specialty care disciplines.
• Conduct intake assessments to identify member needs, treatment barriers, and care coordination opportunities within the LPN scope of practice.
• Educate members on infusion benefits, authorization requirements, site-of-care options, treatment scheduling, and available support resources.
• Coordinate communication among members, providers, infusion centers, specialty pharmacies, payers, and internal Lantern teams to facilitate timely treatment delivery.
• Support benefit verification, prior authorization processes, treatment scheduling, and infusion-related care coordination activities.
• Assist members in understanding financial responsibilities, including deductibles, coinsurance, and out-of-pocket costs associated with infusion therapies.
• Identify and help resolve barriers to treatment, including access, transportation, scheduling, financial concerns, and provider coordination challenges.
• Complete accurate, timely, and compliant documentation of member interactions and care coordination activities within Lantern systems.
• Perform outreach, follow-up activities, and case management tasks within established service-level expectations.
• Monitor infusion-related cases to support timely progression through authorization, scheduling, and treatment initiation milestones.
• Participate in quality assurance reviews, call audits, and continuous improvement initiatives.
• Collaborate with Registered Nurses, Care Advocates, Medical Records Specialists, Client Success partners, and other cross-functional teams.
• Contribute to team meetings, workflow enhancement efforts, training activities, and operational improvement initiatives.
• Adhere to Lantern workflows, applicable regulatory requirements, clinical protocols, and documentation standards.

Requirements :

• Active Licensed Practical Nurse or Licensed Vocational Nurse (LPN/LVN) license in good standing.
• Minimum three years of healthcare experience.
• Minimum two years of experience in infusion therapy, oncology, specialty pharmacy, care management, utilization management, or a related healthcare specialty preferred.
• Strong understanding of medical terminology, healthcare benefits, and infusion treatment processes.
• Experience coordinating care and communicating with providers, health plans, pharmacies, or patients in a healthcare setting.
• Proficiency in Microsoft Outlook and Microsoft Office Suite.
• Experience using electronic medical records, care management systems, or CRM platforms.
• Ability to work independently in a hybrid environment.
• Strong written and verbal communication skills.
• Fluent bilingual proficiency in English and Spanish preferred but not required.

Compensation: Base salary range: $75,000-$85,000 + bonus (depends on location/level/relevant experience)
