About the role
We are looking for a Senior Full-Stack AI Engineer to join a healthcare technology project and become the technical anchor of a small, three-person engineering team.
You will take ownership of both the AI agent architecture and the full-stack application, working closely with two other engineers. This is a hands-on senior role combining backend development, AI orchestration, system architecture, and frontend engineering.
The project focuses on building AI-powered clinical workflow automation tools that help healthcare professionals process information and prepare clinical documentation while keeping all critical decisions under human control.
What you'll do
Design and build production-ready AI agent pipelines, including intake triage, chart retrieval, cross-referencing, flag detection, protocol matching, and preparation assembly.
Develop and maintain AI orchestration across models such as Anthropic and OpenAI, optimizing token usage, cost, and latency.
Build and integrate MCP (Model Context Protocol) servers and tool interfaces, ensuring secure and scope-limited access to healthcare data.
Implement robust guardrails and human-in-the-loop review workflows. All patient-facing information and chart updates must go through the appropriate clinical approval process.
Develop the clinician-facing application using React and TypeScript, including nurse workspaces, protocol libraries, and physician summary views.
Build and maintain backend services using Python and FastAPI.
Integrate with healthcare systems and EMR vendor APIs for chart retrieval and draft-only document filing.
Establish monitoring, logging, evaluation, and testing practices for AI agents and application workflows.
Provide technical guidance and coordinate implementation across a team of three engineers.
Requirements
5+ years of commercial full-stack engineering experience.
Strong hands-on experience with Python and FastAPI.
Solid production experience with React and TypeScript.
Proven experience building and deploying AI agentic systems in production, including:Tool use and function calling.
Multi-step agent workflows and pipelines.
Guardrails, validation, and fail-closed mechanisms.
LangChain, LangGraph, or equivalent frameworks.
Practical experience building or integrating MCP servers and designing secure tool interfaces.
Experience delivering software under HIPAA or equivalent healthcare data privacy and security requirements.
Understanding of least-privilege access, audit logging, data protection, and secure system design.
Experience with multi-model orchestration, including routing tasks between different LLM providers based on cost, latency, and capabilities.
Strong architectural thinking and the ability to make technical decisions independently.
Upper-Intermediate or higher English communication skills.
Will be a plus
Experience with speech-to-text systems and transcript-processing pipelines.
Production integrations with EMR systems, FHIR, or other healthcare APIs.
Experience with local model serving, such as vLLM or Ollama.
Familiarity with AI evaluation and monitoring tools such as LangSmith, promptfoo, or custom evaluation frameworks.
Experience building clinical workflow automation or EMR-adjacent products.
Understanding of clinical workflows and the importance of keeping AI-generated outputs subject to physician review.
